  • Upon acceptance of these Terms and Conditions SMTA will provide basic hookup for one TV. Any additional TVs are subject to further installation charges. A monthly fee will apply for each additional cable box provided.
  • Equipment remains the property of SMTA.
  • An early termination charge of $200.00 will apply to subscribers terminating basic service prior to one year.
  • Upon cancellation, equipment must be returned in working order, to the office within 7 days. Customer will be charged replacement cost for equipment not returned to the office within 7 days of terminating service.
  • Additional charges will be applied to the subscriber’s account if SMTA must make a visit to remove cable equipment.
