Easement For Drop Burial – SMTA – Sherwood Mutual Telephone Association

This is your Easement form from Sherwood Mutual Telephone Company. This gives us permission to bury the drop on your property.

On question 1, I need your name and as much info about your property as possible. Color of house, size of property and anything that is buried under ground.

Question 2 is where you will put if you have a specific spot you want your drop buried. As well as, if there is anything buried underground on the property the OUPS will not locate. (i.e. Electric dog fences, secondary gas lines, pool lines, electric lines).
