I acknowledge that I have read The Sherwood Mutual Telephone Association Customer Agreement and accept all terms and conditions. I also understand that use of my Sherwood Mutual Telephone Association Broadband service may be subject to sales tax. Subscribers are billed on a monthly basis on the same bill with their regular local telephone service. (Entering your full name in this field will be considered a valid signature for this contractual agreement.)
I agree to subscribe to the INTERNET service for 12 months (1 year), and understand I will be billed a $200.00 early termination fee if Internet is disconnected before twelve (12) months. (Entering your full name in this field will be considered a valid signature for this contractual agreement.)
This agreement sets forth the terms and conditions for the services provided to the subscriber. Upon acceptance of these Terms and Conditions SMTA will provide services to the customer. An early termination fee of $200 will apply to subscribers terminating service prior to One (1) year under the One (1) year agreement. A truck roll charge of $8 may apply as well as a minimum service fee of $50 to visits after installation. A Protection Plan may be purchased to help protect from incurring additional fees. These prices do not include tax and are subject to change.
